Connect With People Exploring Concierge Healthcare
Concierge Care Guide helps consumers research concierge and membership-based healthcare. Our provider directory lists practices from public information, and healthcare professionals can help keep their practice information accurate.

About the CCG Directory
Concierge Care Guide is an independent consumer resource about concierge medicine, direct primary care, membership fees, insurance, and Medicare. The directory sits beside those guides, so that someone who has learned how the models work can see who offers them.

How Provider Listings Work
A listing starts from public information
We begin with what a practice publishes about itself. A listing shows only details we can attribute to a source, and it leaves out the rest.
Each person and each practice has one record
A physician at two offices has one listing that shows both.
A claimed badge shows association, not quality
When a clinician or practice claims a listing and we approve the claim, our logo appears beside the name. It does not rate care or affect search order, and every plan follows the same approval steps.

Claiming and Updating a Profile
Claiming is not open yet. The steps below show how it is planned to work. Until then, if something on your listing is wrong, or you want it removed, use our contact form.
Find your listing
Search by your name, practice, or city.
Select the correct listing
Choose the provider or practice that is yours.
Request ownership verification
Tell us who you are and how we can confirm it.
Submit updated information
Enter only the details that should change.
Await administrative review
A person reviews the request. Nothing is approved automatically.
Is a listing an endorsement?
No. The directory helps people find information. A listing is not a recommendation, referral, or statement about quality.
How is my NPI used?
An NPI can help identify a provider. It does not by itself establish license status, credentials, concierge participation, or who may manage a profile, so an NPI alone does not approve a claim.
Is my practice listed now?
Search the directory to find out.
What if something on my listing is wrong?
Use the link on your profile, or our contact form. Changes appear after a person has reviewed them.
Why is my practice listed when I did not ask?
We list practices from public information, mainly their own websites. You can ask us to correct or remove your listing at any time through our contact form.
Do you collect patient information?
No. The directory has no patient accounts or patient records.
